조기위암 내시경 치료 후 scar의 모양은 참으로 다양합니다. 시간에 따라 모양이 변하기도 합니다.
위각의 거의 전부를 차지하는 비교적 큰 융기형 조기위암에 대한 내시경 치료를 시행하였습니다.
ESD: Early gastric carcinoma
1. Location : angle, lesser curvature
2. Gross type : EGC type IIa+IIb
3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : intestinal
5. Size of carcinoma : (1) longest diameter, 58 mm (2) vertical diameter, 24 mm
6. Depth of invasion : invades mucosa (lamina propria) (pT1a)
7. Resection margin : free from carcinoma(N), safety margin : distal 8 mm, proximal 7 mm, anterior 10 mm, posterior 10 mm, deep 1800 ㎛
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent
2개월 후 첫 내시경 검사에서 ESD ulcer scar가 붉고 불규칙하게 융기된 granulation tissue 처럼 보여 향후 융기형 scar가 될 모양이라고 추정했습니다. 그런데 반년 후 다시 살펴보니 아주 flat하고 예쁜 scar가 되어 있었습니다. Scar의 모양이 어떻게 변할지는 그야말로 알 수 없는 일입니다.
