1. Proximal gastrectomy with esophago-jejunostomy 인기가 식은 이유

제가 2004년 처음 부교수 발령을 받았을 무렵에는 간혹 proximal gastrectomy with EG-stomy가 시행되었습니다. 술기가 간단하여 외과 의사에게는 쉬운 수술이면서, 환자는 위가 남는다는 안도감을 가질 수 있었습니다. 그런데 위가 남는다는 것이 꼭 좋은 일만은 아닙니다. 두 가지 이유입니다. (1) 협착과 역류성 식도염, (2) 잔위의 종양이 그것입니다.

Proximal gastrectomy 후 협착은 balloon dilatation으로 해결할 수 있습니다. 그런데 역류성 식도염은 정말 조절하기 어렵습니다. 온갖 약을 다 써도 호전되지 않는 경우가 있으며, 결국 total gastrectomy까지 해 주는 경우가 있습니다.

위암은 원래 원위부에 흔합니다. 근위부 위암 환자에서 암 부위만 제거하고 원위부 위점막을 남겨둔다는 것은 metachronous neoplasm의 위험이 있습니다. 아래 환자는 proximal gastrectomy 후 잔위 체하부 소만에 3.6cm 고도선종이 발생하여 의뢰된 경우입니다. ESD + ablation으로 치료하였습니다만 접근이 어려워 상당히 힘들었습니다.

최근에는 proximal gastrectomy with EJ stomy의 술기에 몇 가지 변화를 적용하여 다시 시행되는 경우가 있습니다.

Proximal gastrectomy에 대한 Pros and Cons 찬반토론이 있다는 소식을 듣고 2015년 5월 23일 (토) 강남세브란스병원에서 열린 '2015년 대한종양외과학회 연수강좌'에 다녀왔습니다.

서울대분당병원 박도중 선생님 발표:

과거 proximal gastrectomy with esophagogastrostomy 후 reflux esophagitis, stricture와 같은 후기 합병증이 많아서 최근에는 total gastrectomy가 대세입니다. 외과 교과서에도 proximal gastrectomy를 추천하지 않고 있습니다. 최근 분당서울대병원에서는 근위부 조기위암에 대하여 laparoscopic proximal gastrectomy (LPG) with double tract reconstruction (DTR)을 시행하고 있습니다.

식도와 위 사이에 10-15 cm 간격을 유지하면 역류성 식도염은 큰 문제가 되지 않는 것 같습니다. 51예의 LPG with DTR 증례를 laparoscopic total gastrectomy와 후향적으로 비교해 보았는데, reflux esophagitis가 거의 발생하지 않았고 빈혈도 적었습니다. 현재 전향적 연구인 KLASS-05연구를 진행하고 있습니다.

아주대학교 허훈 교수님 발표:

Oncologic safety가 중요합니다. Proximal gastrectomy에서는 5번, 6번 node dissection를 못하는데 이 부위는 EGC에서는 거의 문제가 없습니다만 AGC에서 1% 정도 림프절전이가 있습니다. 수술전 EGC로 생각했으나 수술 후 AGC로 나온 환자가 가능하므로 주의해야 합니다. 게다가 남아있는 distal stomach의 암 발생에 대한 경계를 늦추면 안 됩니다.

Laparoscopic proximal gastrectomy (LPG) with double tract reconstruction (DTR)에 대한 다기관 전향적 연구(KLASS-05)가 진행되었고 2023년 발표되었습니다 (JAMA Open 2023). 축하합니다. 결론 부분을 옮깁니다.

"This multicenter RCT showed that patients with upper-third early GC who received LPG-DTR required less vitamin B12 supplementation than those who received LTG, with no increase in complication rates and no difference in overall and disease-free survival rates. There was no difference in change in hemoglobin levels between the groups. In addition, the LPG-DTR group had better scores on physical and social functioning scales than the LTG group. These findings suggest that LPG-DTR may be as safe as LTG and a function-preserving procedure for the treatment of patients with upper-third early GC."

High body (cardia 직하부) 암 환자입니다 (위암 707). Proximal gastrectomy with double tract reconstruction 수술을 권하였습니다. 결과는 잘 나왔습니다.

4. Proximal gastrectomy with DTR 후 remnant distal stomach의 관찰

위를 일부 남기기 위해서는 proximal gastrectomy 후 jejunal interposition이나 double tract reconstruction을 해서 식도와 위가 바로 만나지 않게 하면 좋습니다. 저희 기관에서는 double tract reconstruction (DTR)을 선호하고 있는데요... 문제는 total gastectomy 후 E-J stomy와 double tract reconstruction 후 E-J stomy가 거의 비슷해 보이므로 내시경의사가 remnant distal stomach을 관찰하지 않는 경우가 있다는 것입니다. DTR 수술인 것을 알고 remnant distal stomach을 관찰하려고 해도 접근이 어렵거나 anatomy가 헷갈리는 경우도 많습니다.

DTR 수술 환자의 내시경 검사를 증례로 소개하겠습니다.

한가지 걱정스러운 점은 proximal gastrectomy with EJ 후 screening은 어떻게 될지 궁금합니다. 안쪽으로 들어가서 관찰하기가 어렵지는 않을지...

좋은 질문 감사합니다. 일견 total gastrectomy 후 위를 관찰하는 것과 비슷할 것 같다는 생각이 듭니다. 그러나 상세한 것은 알 수 없어서 분당 서울대병원 박도중 선생님께 문의하여 아래와 같은 답변을 받았습니다.

이준행 교수님. 위내시경 하는 데는 특별히 문제가 없습니다. 다만 반드시 잔위를 봐달라고 검사오더 비고란에 표기하고 있습니다. 만일 이러한 코멘트가 없으면 소화기내과에서 위전절제 한 줄 알고 잔위를 안볼 수 있기 때문입니다. 감사합니다.

외과-내과 사이 소통만 잘 되면 문제가 없다는 말씀 같습니다. 외과 환자가 정확히 어떤 수술을 받았는지 잘 알고 수술하는 것은 내시경 의사의 기본이라고 생각합니다. 항상 쉬운 일은 아니겠지만...

안녕하세요, 이준행 교수님 !

오랜만에 proximal gastrectomy에 관련된 내용이 와서, 이전에 혹시나 도움이 되었으면 하고 저장해두었던 사진을 보내드립니다.

이 환자의 경우 2년 정도는 comment에 잔위나 proximal gastrectomy에 대한 언급이 없어서 이전 검사에서는 total gastrectomy처럼 내시경 사진이 찍혀 있었는데, 그 이후에 comment에 proximal gastrectomy 언급이 있어 교수님의 메일이 생각나 조심스럽게 진입해서 들어가 보았고, 당시에 신기해했던 케이스입니다.

제가 처음 본 proximal gastrectomy with DTR 내시경 사진이기에 저장을 해놓았다가 잊고 있었는데 이번에 간단하게 사진 정리를 해보았습니다.

이 환자의 경우 잔위가 좀 작은 편이고, bile reflux로 인한 change가 좀 심한 편이지만, 그래도 혹시나 글로만 보다가 이런 케이스를 직접 내시경적으로 보면, 여러 독자들이 혹시나 도움이 될 듯 하여 보내드립니다.

그렇습니다. 좋은 내시경 검사를 위해서는 좋은 정보가 필요합니다. 내시경을 의뢰하는 분은 충분한 정보를 주셔야 합니다. 내시경 시술자는 환자의 임상 정보를 잘 확인한 후 시작해야 합니다. 서로 노력합시다.
