1. Mass with deep ulceration: similar to AGC B-II

큰 mass이고 중앙에 넓은 궤양이 있습니다. 궤양 옆의 융기부는 BGU에서 보이는 edematous margin이 아니라 궤양을 둘러싸고 있는 견고한 heaped up margin입니다. AGC B-II 치고는 너무 크고, AGC-B-III 치고는 너무 깨끗합니다. AGC가 이만큼 큰 경우는 보통 좀 더 infiltrative한 양상을 보이는 경우가 많습니다. 그래서 lymphoma를 의심해야 합니다. (목요점심집담회 20160602)

임신 도중 발견된 위 림프종 (2013). Borrmann type II와 유사한 부위가 있지만 주변으로도 infiltration한 양상과 작은 궤양도 있음.

보만 4형 진행성 위암과 유사한 Diffuse large B cell lymphoma의 내시경적 특징

(1) 주름의 비후가 less homogeneous하고 (두께의 차이가 있다는 의미)
(2) 주름은 비후되어 있으나 골짜기는 유지되는 편이며
(3) 표면이 pale하면서 매끈하고 탱탱한 느낌이며 moasic pattern을 보이지 않으며
(4) ulcer나 erosion이 드물고
(5) 공기를 주입하면 잘 펴지는 편이다.

위는 diffuse large B cell lymphoma의 infiltrative type인 것으로 이해하면 무난할 것 같습니다. 십이지장 구부의 붉은 부위까지 DLBCL로 나왔습니다.

F/35. Melena로 검사를 받았고 high grade MALToma라는 조직소견으로 의뢰되었으나 내시경 재검에서 diffuse large B cell lymphoma로 확인되어 R-CHOP 시행
SMT 형태의 gastric lymphoma
병리 진단. LARGE B-CELL LYMPHOMA
Differential diagnosis; 1) diffuse large B-cell lymphoma, not otherwise specified, activated B-cell type, 2) T-cell/histiocyte rich large B-cell lymphoma (2018 SMC 소화기병심포지엄 민양원 교수님 강의)
첫 조직검사에서 undifferentiated malignant tumor, prefer sarcomatoid carcinoma로 나와 수술을 하였고 DLBCL로 확인되었음 (2015, M/55). DLBCL 병소가 두 곳이고 림프절 전이가 있었음
위암 619
8년전 hilar mass (diffuse large B cell lymphoma)로 항암치료 받고 CR이던 환자의 gastric recurrence. 위 림프종은 간혹 Borrmann type II와 구분이 어렵습니다. 이 환자는 gastric biopsy에서도 diffuse large B cell lymphoma가 나왔습니다.
