1. History of Korean Antireflux Surgery Study Group (KARS) - 아주대 한상욱

KARS의 전회장님이신 아주의대 한상욱 선생님께서 KARS 의 역사를 소개하셨습니다. 최근 대한위암학회 산하의 연구회로 승격하여 처음으로 학술대회를 개최한 것에 대한 소회를 말씀하셨습니다.

미국에서는 1년에 2만 건의 항역류수술이 시행되는데 반하여, 우리나라에서는 2014년에 약 45건의 항역류수술이 시행되었다고 합니다.

위식도역류질환 가이드라인(2012) 작성 작업에 몇 분의 외과 선생님께서 참석하셨다고 합니다. 23번 권고문이 항역류 수술에 대한 것입니다.

2. How I implemented antireflux surgery - 가톨릭대 김진조

회장이신 김진조 선생님의 강의는 이런 말로 시작하고 있었습니다. My life is too long for just gastric cancer! I decided to be a true foregut surgeon.

전형적인 증상, 특히 regurgitation이 심한 환자에서 수술의 효과는 좋습니다. 내과에서 전과된 환자가 아닌 스스로 정보를 찾아보고 외과의사를 찾는 환자는 주의해야 합니다. 나름 확신을 가지고 있으므로 문진을 하더라도 환자 스스로 수술에 적합한 정답을 이야기하기 때문입니다. 문진이 정확하지 않다는 것이지요.

3. High resolution manometry - 고신의대 박무인

High resolution manometry는 전통적인 manometry에 비하여 물을 사용하지 않으므로 검사 자체가 쉽습니다. 아울러 결과 해석이 쉽고 interobserver variation이 적어 유용합니다.

박무인 교수님은 주로 올해 나온 Chicago classification version 3.0을 중심으로 말씀해 주셨습니다 (Kahrilas. NM 2015).

4. Impedence test for GERD - 가톨릭대학 조유경

Conventional pH 검사의 시대는 가고 impedence pH 시대가 왔습니다. 물론 Bravo도 가능하지만 pH 정보밖에 얻을 수 없다는 단점이 있으며 현실적으로는 수입이 중단되어 시행이 어렵습니다. Impedence 검사 결과는 증상과 연관지어 해석해야 합니다. 비록 정상이라는 결과라고 보고되더라도 증상과 잘 matching 하면 진단의 단서를 찾기도 합니다.

5. 초청연자 강의 - Guilherme M Campos (Virginia Commonwealth University)

미국사람들이 heartburn이라고 해도 그 의미가 다르므로 환자의 진짜 증상이 무엇인지 잘 따져봐야 합니다.

New technologies - Campos 박사는 약물치료와 항역류수술의 중간단계 치료로 간주하고 있었습니다.

1) Stretta (low frequency RFA)
2) Transoral incisionless fundoplication (TIF)

4) EndoStim therapy (LES electrical stimulation)
5) MUSE system

Rise and fall of anti-reflux surgery in US

Spechler 논문이 두번 나온다는 점이 흥미로웠습니다. 경험이 없는 Surgeon이 많다.

6. POEM - 연세대학교 정현수

Anterior vs Posterior myotomy
- Anterior myotomy: continuing toward the lesser curve of the stomach potentially avoides damage to the sling collar muscle, which is major component of the angle of His → to minimize post-procedure reflux (Angle of His가 유지된다는 의미)
- Posterior myotomy: better relief of dysphagia because of sling fibers but at the expense of mild reflux (Angle of His가 망가진다는 의미)

2015년 8월까지 연세대학교(신촌+강남)에서는 POEM 22예를 시행하였다고 합니다. 성적은 매우 좋았습니다.

Patel 등이 분석한 가장 최근까지의 자료입니다 (Patel K. Dis Esophagus 2015). 다 좋은데 한 명이 죽었군요.

Minor operative adverse events included capno/pneumo-peritoneum (30.6%), capno/pneumo-thorax (11.0%) and subcutaneous emphysema (31.6%). Major operative adverse events included mediastinal leak (0.3%), postoperative bleeding (1.1%) and a single mortality (0.09%). There was an improvement in lower esophageal sphincter pressure and timed barium esophagram column height of 66% and 80% post-POEM, respectively. Symptom improvement was demonstrated with a pre- and post-POEM Eckardt score ± standard deviation of 6.8 ± 1.0 and 1.2 ± 0.6, respectively. Pre- and post-POEM endoscopy showed esophagitis in 0% and 19% of patients, respectively.
