Screening endoscopy is very important for the early detection of gastric cancer. In Korea, gastroscopy every other year is recommended for every male and female above 40 years. 90% to 100% of the medical cost of the national screening program is covered by the government fund. However, the screening participation rate is around 45% in the national program and around 30% in the private program. 25-30% of the population never get the screening endoscopy before symptom develops.

A Borrmann type IV was found in a first-in-the-lifetime endoscopy of a 59 years old female. Diagnostic laparoscopy showed diffuse pertineal seeding.

Screening program in Korea is world famous. But, there is a room for improvement.
