[Pill-induced esophagitis, 약제성 식도염, 약인성 식도염]

Location (common site): 2nd physiologic narrowing point

Cause drugs: antibiotics(tetracycline, doxycycline, clidamycin), aspirin, potassium chloride, ferrous sulfate, quinidine, bisphosphonates, steroid & NSAIDs…

Symptome: acute onset of chest pain, odynophagia, hearburn…
* associated with a potentially injurious medication taken incorrectly

Diagnosis: history + endoscopy

Prevention
1) Encourage at least 100 mL of water after swallowing medication
2) a preliminary swallow of water before medication
3) standing or sitting upright for at least 5~10 minutes following drug administration

[Cases]

두드러기로 피부과 약을 딱 한번 먹고 epigastric pain으로 내원한 젊은 여성입니다. 상절치 38 cm 부위에 irregular circumferential ulcer가 있었고 조직검사에서 CMV나 HSV는 음성이었습니다. 곧 호전되었습니다.
감기약 먹고 이틀 후 심한 odynophagia가 발생하였습니다. 상절치 25 cm 부터 32cm까지 비교적 길게 circumferential lesion이 있었습니다.
물에 섞어 복용하는 magnesium citrate pure powder를 내원 전일 물에 희석하지 않고 한스푼 정도 먹었음.

Local에서 더 자주 보게 되는 병 중 하나인 pill-induced esophageal ulcer. 은근 종종 보게 됩니다. 전형적인 history가 있을 때 (소량을 물과 약을 먹고 누웠다... 등등) 조직검사는 필요없을 듯 합니다. 교수님은 어떻게 생각하세요?

전형적인 증세라면 local에서 조직검사까지는 필요하지 않을 것 같습니다. 몇 주 후 재검은 좋을 것 같습니다. To make it sure.
