[Gastric cancer 736 - Single erosion]

Single erosion은 암일 수 있습니다. 조직검사 딱 한 점 시행하였고 "atypical glands, suspected tubular adenocarcinoma가 나왔습니다. 암이라고 판단되었고 ESD를 했습니다.
Stomach, ESD: Early gastric carcinoma
1. Location : antrum, anterior wall
2. Gross type : EGC type IIc
3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : intestinal
5. Size of carcinoma : (1) longest diameter, 6 mm (2) vertical diameter, 3 mm
6. Depth of invasion : invades mucosa (muscularis mucosa) (pT1a)
7. Resection margin : free from carcinoma(N), safety margin : distal 14 mm, proximal 13 mm, anterior 14 mm, posterior 14 mm, deep 500 ㎛
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent
[조직검사 4번 후 ESD 하였던 조기위암]
작은 함몰형 병소의 조직검사에서는 false negative가 나오는 경우가 있습니다. 네번째 조직검사에서 위암 suggestive로 나와 ESD를 하였던 증례입니다.
조금이라도 이상하면 다소 tight한 간격으로 재검을 해 보는 수 밖에 없습니다. 보험제도가 합리적이라면 두번째 정도에서 암이 나오지 않았을 때 diagnostic ESD를 해 볼 수 있을 것인데 우리나라에서는 현실적으로 불가능합니다. (참고: EndoTODAY 위암 1)
어마어마한 건보료(건강세)를 내는 국민들의 억울함은 이해할 수 있습니다. 보험이라는 것이 결국 최선의 진료보다는 적정 진료를 목표로 하기 때문입니다.
