환자들은 위암과 고도선종을 제법 다르게 생각합니다. 저는 거의 같은 것으로 생각하는데요...

한 50대 남자 환자에서 고도선종이 나와 ESD를 권유하고 입원장까지 드렸는데 치료를 받지 않으셨습니다. 2년 후 병소는 커진 상태였고 조직검사에서 암이 나와 수술을 하였습니다. 2년 전에 내시경으로 치료할 기회가 있었는데 상당히 아쉬웠습니다.

Subtotal gastrectomy: Early gastric carcinoma
1. Location : middle third, Center at body and lesser curvature, anterior wall
2. Gross type : EGC type IIc
3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : intestinal
5. Size : 2.4x1.6 cm
6. Depth of invasion : invades mucosa (lamina propria) (pT1a)
7. Resection margin: free from carcinoma, safety margin: proximal 3.3 cm, distal 5.2 cm
8. Lymph node metastasis : no metastasis in 29 regional lymph nodes (pN0) (0/29: "1", 0/1; "3", 0/7; "4", 0/6; "4sb", 0/1; "5", 0/0; "6", 0/4; "8a", 0/2; "7", 0/1; "9", 0/1; "11p", 0/4; "12a", 0/2)
9. Lymphatic invasion : not identified
10. Venous invasion : not identified
11. Perineural invasion : not identified
12. Peritoneal cytology : negative
