[Gastric cancer 532 - 약간의 함몰부위가 있는 조기위암]

약간의 함몰부위가 있는 조기위암은 치료방법 선택이 늘 고민입니다. EUS가 믿을만하지 않기 때문에 더욱 그러합니다. 저는 내시경 육안소견에 집중합니다. 아래 환자에 대하여 여러분은 어떤 치료 방침을 결정하시겠습니까? Forceps biopsy 소견은 moderately differentiated tubular adenocarcinoma였습니다.

저는 ESD를 했습니다. 약간의 함몰부가 있지만 주변은 매우 평탄하였다는 점을 고려하였습니다. 점막암으로 추정했다는 말씀입니다. 여하튼 결과는 잘 나왔습니다. 휴~~

1. Location : mid body, postero-lesser curvature
2. Gross type : EGC type IIc
3. Histologic type : tubular adenocarcinoma, well differentiated
4. Histologic type by Lauren : intestinal
5. Size of carcinoma : (1) longest diameter, 8 mm (2) vertical diameter, 7 mm
6. Depth of invasion : invades mucosa (lamina propria) (pT1a)
7. Resection margin : free from carcinoma(N), safety margin : distal 11 mm, proximal 18 mm, anterior 20 mm, posterior 20 mm
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12 Histologic heterogeneity: absent
